Interested and qualified applicants should signify their interest in writing. Attach the following documents to the application letter and send to Deputy Director for Administration NBI V-Tech Tower, Quezon City or through this link HERE not later than 𝗢𝗰𝘁𝗼𝗯𝗲𝗿 𝟮𝟭, 𝟮𝟬𝟮𝟰.
Requirements
1. Letter of Intent specifying the position applied for and the place of assignment
2. Duly accomplished Personal Data Sheet (PDS) – two (2) original copies of CS Form No. 212 revised 2017 with attached Work Experience Sheet and recent passport size picture with complete name tag
and signature over printed name. Picture must be taken within the last six (6) months. Computer generated or photocopied picture is not accepted.
3. Photocopy of Certificate of Eligibility, Board Rating and License (if new applicant and those with expired licenses)
4. Authenticated copy of Transcript of Records (TOR) (if new applicant)
5. Certified True Copy of College Diploma (if new applicant)
6. Certificate/s of relevant training/s
7. Original Certificate of Live Birth (if new applicant)
8. Original copy of Certificate of Marriage, if applicable (if new applicant)
9. Original NBI Clearance
10. Original court clearances from RTC and MTC and Clearance from the Office of the Prosecutor where applicant resides (if new applicant)
11. Individual Performance Commitment Review (IP’CR) – Year 2023 (1st and 2nd semester) (if applicable)
