LATERAL ENTRY PROGRAM FOR PNP HEALTH SERVICE

Good News to all Profissional Applicants who want to join the PNP Lateral Entry For PNP Health Service.

The PNP Health Service is NOW accepting applications for the Lateral Entry Program for Technical Officers. The following are the requirements:

In-Service (Age: Not more than 46 years old at the time of appointment. Age waiver shall not be allowed

A. Civil Service PDS (Revised 2017)
B. Updated Unit/RMD PAIS Generated PDS
C. Updated Service Record
D. Latest Promotion Order
E. Eligibility/ies
F. Diploma/Transcript of Record (authenticated copy)
G. Birth Certificate (PSA Copy with Receipt)
H. Highest Mandatory Training (Training Certificate, Order of Merit, and Declaration of Graduates)
I. Specialized Schooling and/or Training, Training Certificate, Order of Merit and Declaration of Graduates)
J. Latest two (2) consecutive periods of PFT, IPER, ITR, and SALN
K. Affidavit of Undertaking of Non-Pending Case

Civilian (Not be less than 21 or more than 30 years of age; Age waiver may be granted provided, the applicant shall not be less than 20 or over 35 years old)

A. Personal Data Sheet (Revised 2017; CSC Form 212)
B. Birth Certificate (PSA Copy with Receipt)
C. Certificate of Employment (if applicable)
D. Photocopy of two (2) Government-issued Identification Cards with Picture
E. Certificate of Eligibility
F. Diploma/Transcript of Record (authenticated copy)
G. Related pieces of training Required by the Technical Units Concerned
H. Certificate of Non-Pending Case (if presently employed)
I. Clearances from:
a) Barangay
b) Philippine National Police
c) Directorate for Investigation (DI)
d) Mayor
e) Prosecutor
f) MTC
g) RTC
h) NBI
